Introducing the H.A.R.T

Idaho faces challenges meeting the needs of individuals with mental illnesses severe enough to prevent independent living but not severe enough to require hospitalization. Many currently reside in Residential Assisted Living Facilities (RALFs), which aren't ideal for addressing their complex needs. To bridge this gap, the Home for Adult Residential Treatment (HART) Model offers a new level of care. HARTs provide a combination of residential and behavioral health services, constant supervision, assistance with daily living, and medication management, along with coping skills and crisis intervention. Unlike RALFs, HARTs have the ability to intervene and provide assistance in-house during mental health crises, ensuring uninterrupted support for residents.

The HART Model aims to provide supported living integrated with behavioral health services for individuals with Serious and Persistent Mental Illness (SPMI). It helps them maintain an independent and safe lifestyle conducive to recovery. By delivering clinical and peer services in the home, the HARTmodel ensures additional staff are available to assist participants and licensed professionals can monitor and manage residents' SPMI symptoms.

Partnership

Weare pleased to announce our partnership with One Love Agency to help deliver services to stay in compliance with the state guidelines of the HART model. This partnership will enhance the quality of care and support we provide to our residents, ensuring they receive the comprehensive services they need to thrive. This care includes:

  • In house Counseling

  • CBRS/casework

  • Peer support

  • Mental health groups

  • Med management

  • 24/7 mental health crisis support
