Lewis & Clark is a warm and welcoming 16-bed community offering personalized Level I, II, and III care. Our compassionate and highly trained team is awake and available around the clock, providing peace of mind for residents and their families. We specialize in supporting individuals with mental health needs, traumatic brain injuries (TBIs), and geriatric care.

Residents enjoy three delicious, dietitian-approved meals each day, thoughtfully prepared to encourage healthy and safe eating habits. Engaging daily activities foster connection, creativity, and joy, helping residents feel active and fulfilled.

With an on-call nurse available at any time and open communication with each resident’s care team, we ensure consistent, attentive, and reliable care.

Up and Awake 24-Hour Staff.
This is a 16-Bed Facility and due to our level of care we have a very low turnover rate.

Please contact the Administrator for availability.

We also offer a complimentary "Free Day Stay" when contemplating placement in one of our facilities (pending bed availability). This is a great way to make sure our facility is a "good fit". We want residents to feel comfortable with the staff and atmosphere of their new home.
